Before AB 460, imaging centers with multiple locations often struggled to maintain consistent radiologist coverage. Physicians were required to be physically present at each facility during contrast procedures, creating staffing bottlenecks and limiting appointment availability. According to ContrastConnect, rural and underserved communities were disproportionately affected, with patients in some areas facing longer travel distances and reduced access to timely contrast-enhanced imaging.
AB 460 addresses these issues by aligning California's supervision requirements with federal CMS standards and permitting virtual oversight through real-time, two-way audio and high-definition video. Under the updated rules, supervising physicians must have immediate access to patient records and current imaging studies. Facilities are also expected to maintain documented contingency plans for technology failures, including procedures for arranging in-person supervision if a virtual connection is interrupted.
Patient safety remains central to the updated framework. Centers using virtual supervision must ensure at least one qualified staff member is physically present and trained to recognize and manage contrast reactions, with emergency medications and equipment readily accessible at all times.
For imaging centers, the new framework enables more flexible scheduling and makes it easier to extend operating hours without requiring a radiologist to be physically present for every contrast procedure. Facilities may offer more appointment slots and manage radiologist resources more efficiently across multiple sites. Radiologists also benefit, as the virtual model allows them to supervise procedures across multiple facilities without traveling to each site.
